Hello everyone, I'm Dr. Hyunun Cho and I work at Hwamyeong Jangsiwon Colorectal Surgery Clinic. This week I want to talk to you about something called BIOFEEDBACK TREATMENT.
What is biofeedback?
Biofeedback is a neuromuscular reeducation tool therapists can use to tell if certain processes in our bodies are working correctly. It is a painless process that uses a computer and a video monitor to display bodily functions of which we are usually unaware. Special sensors measure these functions, which are then presented as audio sounds or as line-graphs on a computer screen. In this way, we can receive information (feedback) on how our bodies work.

A therapist can also help us use this information to modify or change abnormal responses into more normal patterns. This may mean increasing a response, decreasing a response, or learning to coordinate two responses more effectively.

How can biofeedback help?
Bowel control is a bodily function that can be shaped by biofeedback. Bowel control is something most of us master in our early childhood years. Once control is established, we think very little about these processes. That is, until something goes wrong. Disease, trauma, or age-related changes in the body may interrupt our ability to command this crucial body function. Biofeedback techniques help us reestablish these learned patterns. The technology allows us to retrain defective processes, and to restore more normal function.

Biofeedback and home strengthening exercises are non-surgical and non-invasive therapy options that have been shown to help relieve many of the problems people experience. Participating in these types of therapy can help restore a sense of personal control. However, the benefits of biofeedback vary from person to person.

What disorders may be treated through biofeedback?
Several bowel disorders such as incontinence, constipation, and painful spasms of the pelvic floor muscles, can all be treated.

When should biofeedback therapy be used?
It is important to consult a qualified physician when considering treatment options. One or more of a variety of diagnostic tests may be necessary. Before treating for incontinence, a thorough assessment of the muscles and nerves in the pelvic floor should be carried out. In order to implement a neuromuscular reeducation program that best fits one's needs, detailed information relating to his or her symptoms needs to be given to the physician and therapist. Once an evaluation has been made, the correct treatment option can be implemented.

Who administers biofeedback therapy?
Biofeedback is administered by qualified physicians, nurses, or physical or occupational therapists. Although it can be very helpful, it is not available in all medical centers.

A few specialist centres offer biofeedback therapy, or in other words, bowel and muscle retraining. This treatment usually involves an initial assessment by a specialist nurse or physiotherapist who obtains a detailed assessment of your bowel symptoms, overall health, diet, lifestyle and any other factors that may influence your bowel function

From this assessment and the results of any other investigations you may have had, the therapist will discuss with you the likely cause of your problems and set an individualized treatment program for you to follow.

How is BIOFEEDBACK TREATMENT administered?
Biofeedback involves placing sensors on specific parts of your body in order to give you feedback as to how they are functioning. Depending on the treatment target, biofeedback measures such things as heart rate, muscle tension and sweat activity. You can then use these measurements to bring about desired changes

How is BIOFEEDBACK used to help people?
Biofeedback is a behavioral approach that can be used by some people with severe chronic constipation. During defecation, the muscles of the pelvic floor and external anal sphincter should relax as a person bears down. Biofeedback can be helpful in treating people who involuntarily squeeze (rather than relax) these muscles (known as dyssynergic defecation).

"A visual monitor is used to measure external anal sphincter pressures while bearing down. The patient watches a recording of the muscle activity and is asked to change their responses through trial and error."

This excerpt suggests that biofeedback may be a viable treatment option for you if your chronic constipation is severe and you appear to suffer from dyssynergic defecation. Dyssynergic defecation is a condition involving the nerves and muscles in your pelvic floor - the part of your body that supports the organs in your lower abdomen. When a person suffers from dyssynergic defecation, the muscles in the pelvic floor and the anus do not relax as they should during a bowel movement.

Biofeedback for dyssynergic defecation involves retraining your pelvic floor muscles. Although painless, the procedure is certainly awkward. Small sensors are inserted within the anus, while other sensors may be placed within your vagina (if female) or on your abdomen. You will be asked to bear down as if you are having a bowel movement. As you do so, you will see a graphic display of your muscle tension on a display screen. This feedback will allow you to change the way your muscles respond, so that you learn to consciously relax the muscles in this area when you wish to initiate a bowel movement. Your biofeedback treatment is usually complemented by home practice with kegel exercises.

How long does biofeedback therapy last?
Therapy sessions may vary in number and length. A person with a non-neurological problem may require an average of six to eight treatment sessions over a three-month period. These sessions usually occur on a weekly basis and decrease in frequency as the patient improves.

Practice is the key to real improvement. Patients need to take what they learn in therapy sessions and apply the knowledge at home. Accurate repetition and application of the patterns to everyday situations is critical to reestablishing bowel control.

What is the result and success rate of Biofeedback tretment ?

Constipation is not something that people like to talk about but it can become a serious problem if it's not dealt with properly. I'm going to start today by asking you some rhetorical questions that will let you find out how much you really know about this issue.
-1st , Do you know constipation can make cancer in colon? the answer is yes, long time constipation, feces contain many bacteria and carcinogen and they have increasing chance to contact of carcinogen and colonic mucosa, so one of the most common cause of colon cancer in woman is constipation.

-2nd , Did you heard long time constipation can make incontinece in anus? the answer is yes, you know during constipation , long time straining to defecate makes damage to nerve and it weakens the anal sphincer and the result is incontinence.

-3rd , What do you think constipation can make irreversible nerve damage in colon? the answer is yes and long time constipation and intake of constipation drug makes damage to nerve and it aggravate the constipation.

-4th, Did you heard constipation make discoloration and aging of skin ? you know it's terrible but the answer is yes, long time constipation makes toxic material in feces enter the blood stream and it cause skin discoloration and aging.

-5th , What do you think that estrogen hormone make constipation in woman? the answer is yes, so many woman feel constipation after menstration, and low intake of water, low activity, low food intake, low exercise, pregnance and delivery are another cause of constipation. So woman is 3 times more frequent constipation than man. So, 30% of Korean woman intake constipation drug(laxatives).

-6th ,Did you heard constipation may have hereditary trannsmission from parents to his children? If one of identical twins is constipation, the other is 4 times greater chance to have constipation. And not only hereditary factor , if parent's dietary habit is low intake fiber and food , their children's dietary habit is usally same, so they easily get constipated.

Many people think that constipation is only symptom not a disease and intake of constipation drug can cure the constipation , but if constipation make disease even cancer and in America, they spend 2 trillion won to buy constipation drug annually , and constipation drug makes aggravate constipation , do you still think constipation is a just symptom?

If you want to prevent, or treat, constipation you first have to understand what it really is. Far too many people have a lot of misconceptions about it. The international definition of constipation is 5 symptoms. If you have 2 more symptoms among 5 symptoms for more than 2 months, than you have chronic constipation.

1st is infrequent defecate less than 2times per week
2nd is residual symptom after defecate
3rd is small amount of stool less than 30 gram
4th is prolonged and hard to defecate
5th is very firm and hard stool
6th is no desire to defecate after 2-3 days
This defenition is very important because this may change your conception about constipation

For example, if one person defecate 3-4 times a day, but he doesn't feel complete evacuate feeling , this person is also in the range of constipation.

We can think that Normal bowel habit is Regular bowel movements more than 3 times/week and complete evacuate normal formed stool.
I'm sorry talking about stool in the morning but this is very important.

You know, chronic constipation can result serious effect like yesterday's talking , but also if you didn't defecate this morning , you may have several times to endure defecate desire, this is very suffering and this cause damage to your anal sphincter and colon. Then let's think together "what is the cause of constipation?"

Most common cause of constipation is dietary problem like low intake of food, water,and you know fiber. But, there are many other facters to cause chronic constipation.

For example, diabese, thyroid disease, stress, psychologic problem, long time use of laxatives, colonic tumor and cancer, colonic inflammation and stricture, many other drug like hypertensive drug, opioides, contraceptive drugs can be the cause of chronic constipation.

Did you have breakfast today? You can do a lot to prevent constipation by having breakfast everyday. This is because your bowels are more active in the morning than at any other time of day. Right after you wake up, your body starts to produce a large amount of digestive enzymes and this gets secreted into your stomach and bowels. So if you don't eat breakfast, your bowels aren't doing anything, and all those acidic juices can end up injuring your stomach.

There are two types in constipation broadly Acute and chronic constipation Acute constipation is very short period constipation lower than 2or3 months. This can be relieved by just regular diet, exercise, high intake of water and fiber. But Chronic constipation this is not relieve by diet mangement so you should think to try find the cause and type of chronic constipation.

It may be afraid and bothering but you should go to the specialist and discuss and treat the constipation. In chronic constipation this divide to 3 types of constipation

1st is chronic constipation due to other disease like thyroid ,diabease
2nd is chronic constipation due to mechanical obstruction like cancer, tumor or inflammation.
3rd is chronic constipation due to functional disorder in bowel and rectum

Among thease types, the most common type is chronic constipation due to functional disorder.
This functional constipation also consisted of several functional disease like rectocele, rectal intussusception, perineal descent sydrome, Nonrelaxating puborectalis sydrome.
You may be complicated at this moment, but this is very easy, all of this functional constipation comes from long time straining to defecate in toilet.
You know after long time straining, your bowel may be loosening and dilate due to straining. So rectocele means dilated rectum and perineal descent sydrome means loosening of bowel and sphincer of anorectum.
It's very easy, so long time straining is not good for your health and your anorectum

The most important factor in ensuring a successful diagnosis is for the person suffering from the condition to actually realize that he or she is constipated. Far too often, people suffer from anal pain, abdominal discomfort, bloating, and even anal bleeding without realizing why they're suffering.
You know these symptoms are the result of constipation, because hard stool in bowel make thease result.

First of all we should know he or she is in the range of constipation and the duration, and abdominal symptom like bloating , pain, how long sitting in the toilet , feeling of residual symptom, stool calber and consistency ext...

After these history taking, they will know they have constipation and it's severity and then decide to treat constipation.

Then 2nd diagnostic study is colonoscopy to find out tumor or cancer which obstruct the colon.
If the result is no problem, 3rd diagnostic study is total colonic movement study and defecography to find out functional constipation.

With this study they can see their enlarged bowel, and they know why they cannot defecate easily.
This is very important , because the treatment is different according the type of constipation.
If constipated patient's rectum is so large that stool is remained in enlarged rectum and this inhibit complete defecate.
In this condition we need medical and surgical treatment in hospital.

And 4th diagnostic study is anal manometry study Long time constipation make nerve injury to anus and it makes incontinence state, so we study the anal sphincter weakness and push strength.

And 5th diagnostic study is anal ultrasonographic study we study with this equipment to find out anal sphincter injury

It's may be hard to understand but we should know constipation is not just symptom but disease and can make serious result.

Today I will talk about the "treatment and prevention of constipation"
Many people think that constipation drug can cure the constipation and thease drug has no adverse effect. But some stimulous laxatives like bisacodyl, bicoside, aloe may relieve symptom incidentally , but thease drug have addictive effect and eventually aggravate the constipation .
So we should know that the treatment of constipation is different to their types and not only medical treatment but also surgical treatment.
There is a drug that has no adverse effect for example, psyllium , fiber . This psyliium absorb water inside the colon and expand 30 times more than it's original state and make formed stool, and it also regulate bowel movement easily.
According to the type of constipation , there is functional constipation that cannot cure with medical treatment like rectocele, rectal intussusception, thease disease should be treated combined with surgical treatment. And most common cause of chronic functional constipation is abnormal defecation habit like strainning in toilet. If we have abnormal defecation habit this cannot cure by medical tretment or other supportive treatment.
Thease abnormal habit can be cure by visualizing their defecation habit by computer system and special equipment. With this device the physician advice right defecation movement. This treatment called "Biofeedback treatment" . With this biofeedback treatment , we can treat functional constipation patients with 80% success rate.

Then, How can we prevent constipation?
You know regular exercise more than 30 minutes, high intake of water more than 2 liters,high intake of fiber more than 14 grams,and regular diet with boiled rice with cereals , do not restrain at defecate desire, do not intake lot of instant food , take a good and adjustable sleep about 7 hrs, this is important for regular bowel movent , like to eat fresh friut before diet, this is good for bowel movent and prevent obesity, and don't drink lot of milk , in adult there is low enzyme to digest milk so if you drink daily, diarrhea or bowel irritation may occur,so I recommend intake lot of water than milk.
